Complete the hot lunch order form sent home from school. Please include the grade and room for each child. Place an X in the box to indicate the days your child/children will be ordering hot lunch. MENU SUBJECT TO CHANGE WITHOUT PRIOR NOTICE DUE TO AVAILABILITY OF GOVERNMENT COMMODITY. Optional buffet will be offered on certain Fridays of the month for Grades 7 & 8. Please place an X in the appropriate Friday box to indicate if you are ordering the regular lunch or the buffet.

Please return the bottom portion of the order form with your payment in an envelope marked with your child's name, grade room number and WEEK OF THE ORDER (please remember that lunch orders run 2-3 weeks in advance) return it to your child's homeroom teacher. Please make your check payable to ST. CHARLES CAFETERIA.

The order form is due on Monday for lunches for the following week. No late orders will be accepted. If your child is absent on Monday, send in payment with your child as soon as possible. If a child is absent on the day he/she has ordered lunch; call the cafeteria by 10 AM to cancel the lunch (969-4028).

Student lunch are $1.75 Students receiving free and reduced lunches MUST USE THE FORM BELOW AND INDICATE THE NECESSARY CHANGES.
