Player Fee Calculation Form SPRING 2012
Each school year, a CYO Consent Form must be completed (signatures required by student and parent) and turned in to the coaching staff or the school office for any student who participates in a CYO sport. The form is included in the summer mailing packet and is available online here. Submitting this form once per school year is all that is required. CYO sports include Volleyball, Football, Cheerleading, Basketball, Soccer, Softball, and Track.
Documentation of a current physical must be on file in the school office prior to a student participating in any athletic practices or games. The IHSAA physical form is attached here. Only pgs. 1 & 2 are necessary.
All persons interested in coaching at St. Charles are asked to complete and turn in a Coaching Letter of Interest form. Click this link to access the form. ( SC Coach Nomination Form ) Print, complete, and turn in to the school office or to a member of the Athletic Association.
For consideration for the Fall 2011 school year, please have this turned in by May 4, 2011.
